The research question
How does patient-reported burden vary between routine heart-failure visits?
Proposed design
Clinically supervised observational follow-up with licensed disease-specific assessments, brief diaries and separately authorized record linkage.
Illustrative design: 12-week follow-up with optional longer observation; setup and recruitment are scoped separately.
Data to collect
- Breathlessness and fatigue
- Physical and social limitations
- Reported weight and treatment changes
- Care contacts and patient-reported health status
Planned outputs
- Symptom and health-status trajectories
- Patient-reported support needs
- Record-linked event analysis where approved
What we confirm with you
We agree the population, sample size, country, collection tools, analysis plan and budget. Collection starts after the required consent, ethics or other permissions and a signed scope are in place.
- Require clinical cohort confirmation and an escalation pathway
- Verify permissions for selected instruments
- Hospitalization and clinical outcomes require record confirmation
Clinical records, laboratory results, photographs and device integrations are included only when access, collection quality and permitted use have been verified. Self-reported findings are identified as such.
